Management of Severe Hypertriglyceridemia

Introduction to Treatment

  • The American College of Cardiology recommends immediate fenofibrate therapy to prevent acute pancreatitis in patients with severe hypertriglyceridemia, followed by statin initiation once triglycerides fall below 500 mg/dL 1, 2

Immediate Risk Assessment and First-Line Treatment

  • A triglyceride level of 544 mg/dL places the patient at significant risk for acute pancreatitis, requiring urgent pharmacologic intervention with fenofibrate as first-line therapy 1, 2
  • The American Heart Association suggests that fenofibrate will reduce triglycerides by 30-50%, bringing levels below the pancreatitis risk threshold, which is a Class IIa recommendation with Level B-R evidence for patients with severe hypertriglyceridemia (≥500 mg/dL) and ASCVD risk ≥7.5% 1, 2

Lifestyle Interventions

  • The American College of Cardiology recommends restricting total dietary fat to 20-25% of total daily calories for severe hypertriglyceridemia 3, 5
  • Eliminating all added sugars completely is advised, as sugar intake directly increases hepatic triglyceride production 3, 5
  • Complete abstinence from alcohol is mandatory, as even 1 ounce per day increases triglycerides by 5-10% and can precipitate hypertriglyceridemic pancreatitis at this level 3, 5
  • Targeting 5-10% body weight reduction is recommended, which produces a 20% decrease in triglycerides and is the single most effective lifestyle intervention 3, 5

Sequential Treatment Algorithm

  • Once triglycerides fall below 500 mg/dL with fenofibrate, the American College of Cardiology recommends initiating moderate-to-high intensity statin therapy to address LDL-C and reduce cardiovascular risk 3, 5
  • Adding prescription omega-3 fatty acids (icosapent ethyl 2-4 g/day) is considered if triglycerides remain elevated after fenofibrate and lifestyle optimization 1, 2, 3

Special Considerations

  • The American College of Cardiology suggests considering conditions specific to women, such as premature menopause and history of pregnancy-associated disorders, when discussing statin therapy 1, 2, 4
  • Continuing moderate- or high-intensity statin therapy if well-tolerated is recommended even after age 75 in patients with diabetes or established ASCVD 5

Monitoring Strategy

  • Reassessing fasting lipid panel in 4-8 weeks after initiating fenofibrate is recommended 5
  • Monitoring transaminases (AST/ALT) at baseline and periodically, as well as checking creatine kinase and monitoring for muscle symptoms when combining therapies, is advised 3, 5

Treatment Goals

  • The primary goal is to reduce triglycerides to <500 mg/dL rapidly to prevent acute pancreatitis 3, 5
  • The secondary goal is to achieve non-HDL-C <130 mg/dL once triglycerides are controlled 3, 5
  • The tertiary goal is LDL-C reduction based on cardiovascular risk assessment once triglycerides are stabilized 5
