Acute Hamstring Sprain/Strain Management in Athletes
Initial Management
- The American College of Cardiology recommends implementing the PRICE protocol immediately after injury, including protection, rest, ice, compression, and elevation, to reduce pain and swelling in athletes with acute hamstring strains 1, 2, 3
- Ice should be applied for 20-30 minutes, 3-4 times daily to reduce pain and swelling in athletes with acute hamstring strains 1, 2, 3
- Compression wraps should be applied to promote comfort and ensure circulation is not compromised in athletes with acute hamstring strains 1, 2, 3
- Elevation of the injured area can help reduce swelling in athletes with acute hamstring strains 4
- Nonsteroidal anti-inflammatory drugs (NSAIDs) can help reduce swelling and pain after injury and may decrease recovery time in athletes with acute hamstring strains 5
Support Devices
- Semirigid or lace-up supports are recommended over elastic bandages for functional treatment of acute hamstring strains in athletes 4, 5
- Support devices should be considered for continued use during return to activity, especially for athletes with a history of recurrent strains 4, 5
Special Considerations
- Heat application is not recommended for acute hamstring injuries 4
