Rasburicase Administration Guidelines
Critical Pre-Administration Requirements
- The American Society of Clinical Oncology and the European Hematology Association do not recommend routine premedication before rasburicase administration 1, 2
- Screening for G6PD deficiency is absolutely mandatory before rasburicase administration, as it is contraindicated in patients with known G6PD deficiency due to the risk of severe hemolysis and methemoglobinemia 3
- High-risk populations for G6PD deficiency include patients of African American, Mediterranean, or Southeast Asian descent 3
- A known history of anaphylaxis or hypersensitivity reactions to rasburicase or any excipients is an absolute contraindication 3
- Rasburicase is contraindicated in pregnant or lactating females 3
Administration Protocol
- The standard infusion protocol is to administer 0.10-0.20 mg/kg (or fixed dose 6 mg in adults) intravenously over 30 minutes, diluted in 50 mL of normal saline, and administered at least 4 hours before starting chemotherapy 1, 4
- Aggressive IV hydration should be maintained with a target urine output ≥100 mL/hour in adults, and loop diuretics may be needed to achieve this target 2, 4
- Do not alkalinize urine in patients receiving rasburicase, as this increases calcium phosphate precipitation risk without benefit 4
- Do not administer allopurinol concurrently with rasburicase to avoid xanthine accumulation 4
Monitoring During Infusion
- Monitor for signs of hypersensitivity reaction during the 30-minute infusion, including potential serious adverse reactions such as anaphylaxis, rash, hemolysis, methemoglobulinemia, fever, neutropenia, respiratory distress, sepsis, and mucositis 1
- Check uric acid levels 4 hours after the first dose and daily during treatment, and redose if uric acid remains >4 mg/dL at 12-24 hours after the initial dose 4
- Blood samples must be immediately placed on ice to prevent ex vivo enzymatic degradation of uric acid, which causes falsely low measurements 3, 4
