Naltrexone Dosing and Safety Guidelines for Daily Alcohol Drinkers

Standard Dosing Regimen

  • Initiate oral naltrexone at 25 mg daily for the first 1–3 days, then increase to a maintenance dose of 50 mg once daily; the FDA‑approved regimen for alcohol‑use disorder is 50 mg daily for up to 12 weeks, with the option to extend treatment to 12 months based on clinical response. 1

Initiation Strategies

  • Abstinence‑first approach: Guideline recommendations advise that patients be alcohol‑free for 3–4 days before starting oral naltrexone to minimize the risk of precipitating withdrawal symptoms. 3

Hepatotoxicity Considerations

  • Naltrexone carries a dose‑related risk of hepatocellular injury and is not recommended for patients with alcoholic liver disease according to certain clinical guidelines. 4
  • High‑dose studies (up to 300 mg/day) have documented hepatocellular injury in a substantial proportion of participants, underscoring the importance of adhering to the standard 50 mg dose. 5

Monitoring Requirements

  • Baseline liver‑function tests should be obtained before initiating therapy to detect pre‑existing hepatic impairment. 1
  • Serial monitoring of liver enzymes during treatment is advised, especially in patients with known liver disease, to identify any emerging hepatotoxicity promptly. 1

Combination with Behavioral Therapy

  • Naltrexone must be used in conjunction with counseling or behavioral therapy; monotherapy has not demonstrated sufficient efficacy. Evidence from multiple randomized trials supports this combined approach. 6
  • In trials where cognitive‑behavioral therapy was paired with naltrexone, 62 % of participants avoided relapse to heavy drinking compared with 40 % in the placebo‑plus‑therapy group, indicating a synergistic benefit. 5
