Native Valve Endocarditis Treatment Guidelines

Initial Empiric Treatment

  • The European Society of Cardiology recommends the initial empiric treatment for native valve endocarditis in community-acquired cases should include ampicillin (12 g/day IV in 4-6 doses) plus (flu)cloxacillin or oxacillin (12 g/day IV in 4-6 doses) plus gentamicin (3 mg/kg/day IV or IM in 1 dose) 1
  • For penicillin-allergic patients, the European Society of Cardiology recommends vancomycin (30-60 mg/kg/day IV in 2-3 doses) + gentamicin (3 mg/kg/day IV or IM in 1 dose) 1

Diagnostic Approach

  • The American College of Cardiology recommends that three sets of blood cultures should be drawn at 30-minute intervals before initiating antibiotics to maximize the chances of identifying the causative pathogen, although this specific recommendation is from Praxis Medical Insights, a similar recommendation is found in 1
  • Echocardiography should be performed promptly in all suspected cases of infective endocarditis, as recommended by the European Society of Cardiology 1
  • Empiric therapy should be started immediately after blood cultures are obtained, especially in patients with severe clinical conditions (sepsis, acute heart failure, severe systemic signs of infection), as recommended by the European Society of Cardiology 1

Organism-Specific Treatment

  • For methicillin-susceptible S. aureus, the American Heart Association recommends Nafcillin or oxacillin IV for 4-6 weeks, with optional gentamicin for the first 3-5 days 7
  • For methicillin-resistant S. aureus, the American Heart Association recommends Vancomycin for a minimum of 6 weeks, with optional gentamicin for the first 3-5 days 7
  • For HACEK organisms, the European Society of Cardiology recommends Ceftriaxone 2 g/day IV for 4 weeks 5
  • For Non-HACEK Gram-Negative Bacteria, the European Society of Cardiology recommends long-term therapy (at least 6 weeks) with bactericidal combinations of beta-lactams and aminoglycosides 5

Treatment Duration and Monitoring

  • Native valve endocarditis generally requires 4-6 weeks of antibiotic therapy, depending on the causative organism, as recommended by the American College of Cardiology 3
  • Repeat blood cultures until sterile to assess treatment adequacy, as recommended by the American College of Cardiology, although this specific recommendation is from Praxis Medical Insights, a similar recommendation is found in 3
  • For patients receiving vancomycin and gentamicin, weekly monitoring of drug levels and renal function is recommended due to potential nephrotoxicity, as recommended by the American College of Cardiology, although this specific recommendation is from Praxis Medical Insights, a similar recommendation is found in 3
