Methotrexate Management for Tubal Ectopic Pregnancy
Dosing Recommendations
- The recommended single‑dose regimen for tubal ectopic pregnancy is methotrexate 50 mg/m² administered intramuscularly; for a patient with a body‑surface area of approximately 1.6 m² this corresponds to a dose of about 80 mg. 1, 2, 3
- The Royal College of Obstetricians and Gynaecologists (RCOG) Green‑top Guideline specifies methotrexate 50 mg/m² as the standard single‑dose treatment for tubal ectopic pregnancy. 1, 2, 3
Monitoring Protocol
- Serum β‑hCG should be measured on days 4 and 7 after methotrexate administration to assess treatment response. 1, 2, 3
- If β‑hCG falls by more than 15 % between days 4 and 7, weekly β‑hCG monitoring should continue until the level is below 15 IU/L. 1, 2
- If β‑hCG does not decrease by at least 15 % by day 7, a repeat transvaginal ultrasound is advised to exclude fetal cardiac activity or significant hemoperitoneum, and a second methotrexate dose (same 50 mg/m²) may be administered. 1, 2, 3
Expected Success Rates
- Single‑dose methotrexate achieves overall success rates ranging from 65 % to 95 %; 3 % to 27 % of women may require a second dose. 1, 2, 3
- Success is strongly related to the initial β‑hCG level: 81 %–98 % success when the initial β‑hCG is < 1,000 IU/L, versus only 38 % success when it exceeds 5,000 IU/L. 1, 2, 3
Patient Counseling Points
- Patients should avoid alcohol consumption during methotrexate treatment. 1, 6
- Patients should avoid folate‑containing vitamin supplements while receiving methotrexate. 1, 6
- Common, usually mild, side effects include flatulence, bloating, transient elevation of liver enzymes, and stomatitis. 1, 2
