Suture Hemorrhoidopexy Management

Treatment Algorithm for Hemorrhoids

  • Non-operative management is strongly recommended as first-line therapy for all hemorrhoid grades, including dietary modifications and lifestyle changes, according to the World Journal of Emergency Surgery 1
  • Flavonoids may be administered to relieve symptoms in complicated hemorrhoids, as suggested by the World Journal of Emergency Surgery 1
  • Topical muscle relaxants are suggested for thrombosed or strangulated hemorrhoids, as recommended by the World Journal of Emergency Surgery 1

Surgical Management

  • Excisional hemorrhoidectomy is recommended for grade IV hemorrhoids or cases unresponsive to less invasive approaches, according to Gastroenterology 2
  • Stapled hemorrhoidopexy has advantages of reduced postoperative pain, shorter operation time and hospital stay, and faster recovery, but has a higher recurrence rate compared to conventional hemorrhoidectomy, as reported by Gastroenterology 4

Special Considerations for Thrombosed Hemorrhoids

  • For thrombosed external hemorrhoids presenting within 72 hours of symptom onset, excision under local anesthesia is recommended, according to Gastroenterology 2
  • Incision and drainage alone is not recommended, as stated by the World Journal of Emergency Surgery 1

Potential Complications of Surgical Hemorrhoid Treatment

  • Common complications include urinary retention, bleeding, anal stenosis, infection, and incontinence, with rates ranging from 0.03-36%, as reported by Gastroenterology 4
  • Sphincter defects have been documented in up to 12% of patients after hemorrhoidectomy, according to Gastroenterology 4
  • Stapled procedures carry rare but serious risks including rectal perforation and pelvic sepsis, as noted by Gastroenterology 4

Important Caveats

  • Always rule out other causes of rectal bleeding before attributing symptoms to hemorrhoids, as advised by Praxis Medical Insights 3
  • Colonoscopy should be performed if there is concern for inflammatory bowel disease or cancer based on patient history or physical examination, according to the World Journal of Emergency Surgery and Praxis Medical Insights 1, 3
  • Immunocompromised patients are at increased risk for severe infection after rubber band ligation, as reported by Gastroenterology 2
