Estradiol Deficiency in Young Women
Primary (Ovarian) Causes
- Chemotherapy-induced ovarian failure, particularly with alkylating agents and platinum compounds, can cause dose-dependent ovarian damage, with cyclophosphamide equivalent dose (CED) >8000 mg/m² dropping fertility probability below 50%, according to the American Society of Clinical Oncology 1, 3
- Radiation-induced ovarian failure occurs with abdominal/pelvic radiation, with doses as low as 5 Gy affecting ovarian function in postpubertal females, and a sterilizing dose of approximately 16.5 Gy at age 20 years, as reported by the American Society of Clinical Oncology 1, 2, 3
- Hematopoietic stem cell transplantation (HSCT) causes ovarian failure in 44-100% of recipients, due to the conditioning regimen itself damaging ovarian reserve, as stated by the American Society of Hematology 3
- Surgical oophorectomy directly removes estrogen-producing tissue, resulting in severe hypogonadism, according to the American College of Obstetricians and Gynecologists 1, 2
Central (Hypothalamic-Pituitary) Causes
- Functional hypothalamic amenorrhea (FHA) from low energy availability is the most common non-iatrogenic cause, with energy deficit disrupting GnRH pulsatility and suppressing LH secretion, as reported by the Endocrine Society 4
- Eating disorders, such as anorexia nervosa, and female athlete triad/RED-S, can trigger FHA, with low energy availability causing LH pulsatility disruptions, according to the American College of Sports Medicine 5, 6
- Hypothalamic-pituitary damage from tumors, cranial radiation, or neurosurgery can cause hypogonadotropic hypogonadism, with low gonadotropins (LH, FSH) and secondary low estradiol, as stated by the American Association of Neurological Surgeons 1, 2, 3
Diagnostic Considerations
- Measuring FSH and LH can differentiate primary from central causes, with primary ovarian failure showing elevated FSH and LH (>10-20 IU/L) and low estradiol, and central hypogonadism showing low or low-normal FSH and LH (<5 IU/L) with low estradiol, according to the American College of Obstetricians and Gynecologists 1, 2, 5
- Checking for FHA versus PCOS in women with low estradiol, with FHA showing LH:FSH ratio <1 and lacking insulin resistance and elevated androgens, as reported by the Endocrine Society 4
- Hormonal contraceptive use fundamentally alters hormone assessment, with ovarian function unable to be reliably assessed during use, and withdrawal bleeding on oral contraceptives being pharmacologic, not evidence of ovarian function, according to the American College of Obstetricians and Gynecologists 7
