Initiating Anticoagulation in Atrial Fibrillation

Diagnostic Requirements

  • The European Society of Cardiology recommends confirmation by an electrocardiogram to establish the diagnosis of clinical AF and commence risk stratification and treatment, with a Class I, Level A recommendation 1, 2, 3
  • A single EKG showing AF is sufficient to diagnose clinical AF and initiate anticoagulation, according to the European Heart Journal 1, 2, 3
  • The diagnostic threshold requires heart rhythm with no discernible repeating P waves and irregular RR intervals when atrioventricular conduction is not impaired, as stated in the European Heart Journal 4

Immediate Steps After EKG Confirmation

  • The European Society of Cardiology guidelines indicate that if CHA₂DS₂-VASc ≥1 in men or ≥2 in women, anticoagulation is indicated, with a focus on reducing mortality and morbidity 4
  • Patients with stroke risk factors should receive oral anticoagulation, prioritizing reduction of mortality and morbidity, as recommended by the European Heart Journal 5

Essential Baseline Work-Up Before Starting Apixaban

  • The European Society of Cardiology recommends assessing renal function, liver function tests, complete blood count, serum electrolytes, and thyroid function tests before initiating apixaban therapy 2, 3
  • The International Journal of Stroke suggests considering serum creatinine ≥1.5 mg/dL as a criterion for dose reduction 7

Apixaban Dosing Algorithm

  • The standard dose of apixaban is 5 mg twice daily, according to the European Heart Journal 6
  • A reduced dose of 2.5 mg twice daily is recommended if the patient meets ≥2 of the following criteria: age ≥80 years, weight ≤60 kg, or serum creatinine ≥1.5 mg/dL, as stated in the International Journal of Stroke and European Heart Journal 6, 7

Special Populations Requiring Warfarin Instead

  • The European Society of Cardiology guidelines recommend using warfarin instead of apixaban in patients with mechanical heart valves, moderate-to-severe mitral stenosis, or end-stage chronic kidney disease with CrCl <15 mL/min or dialysis, as stated in the European Heart Journal 6

Timing Considerations

  • The European Society of Cardiology guidelines suggest starting apixaban immediately after confirming AF on EKG and completing the baseline work-up, without requiring repeat EKGs or prolonged monitoring, with a Class I, Level A recommendation 1, 2, 3
  • If cardioversion is planned, ensure at least 3 weeks of therapeutic anticoagulation with apixaban before cardioversion, as recommended by the Chest journal 8

Common Pitfalls to Avoid

  • The European Society of Cardiology guidelines advise against delaying anticoagulation waiting for additional EKGs, as a single documented episode is sufficient, and delaying increases stroke risk, with a Class I, Level A recommendation 1, 2, 3
  • The International Journal of Stroke suggests reassessing renal function at least annually, as apixaban dosing depends on accurate creatinine clearance, which changes over time 7
