Management of Neonatal Bleeding

Blood Product Administration

  • The American Society of Anesthesiologists recommends RBC transfusion of 10 mL/kg to increase hemoglobin by approximately 20 g/L in neonates 1, 2
  • Fresh frozen plasma (FFP) should be administered at a dose of 10-15 mL/kg in neonates 1, 2, 3
  • Cryoprecipitate should be administered at a dose of 5-10 mL/kg in neonates 1, 2
  • Platelets should be administered at a dose of 10-20 mL/kg in neonates 1, 2

Special Considerations

  • The American Academy of Pediatrics recommends using CMV-negative blood products for all neonates 1, 2
  • Blood products should be prescribed by volume, not units, in neonates 1, 2
  • Estimated blood volume is 70 mL/kg in children, up to 100 mL/kg in newborns 1, 2, 3
  • Monitoring for electrolyte imbalance and hypothermia is crucial during rapid transfusion in neonates 1, 2, 3

Tranexamic Acid Administration

  • The American Society of Anesthesiologists recommends a loading dose of 15 mg/kg followed by an infusion of 2 mg/kg/h of tranexamic acid for trauma-related bleeding in children 1, 2, 3
  • Weight-based dosing of tranexamic acid is mandatory in infants, and the adult dose of 1 g should not be used 1, 2, 3

Rare but Critical Differential Diagnosis

  • The American College of Chest Physicians recommends administering FFP 10-20 mL/kg every 12 hours or protein C concentrate 20-60 units/kg until lesions resolve in infants with homozygous protein C deficiency, which represents a Grade 1A recommendation 4

Management of Oral Bleeding in Infants

Immediate Stabilization and Treatment

  • If bleeding is severe or the infant is in shock, administer fresh frozen plasma (FFP) 10-20 mL/kg to provide immediate clotting factors while vitamin K takes effect (2-4 hours for response), as recommended by the American Academy of Pediatrics, with a response time that is diagnostic of VKDB 5
  • For homozygous protein C deficiency presenting with bleeding, administer FFP 10-20 mL/kg every 12 hours or protein C concentrate 20-60 units/kg until lesions resolve, according to the American College of Chest Physicians 5
