Olanzapine Dosing Considerations
Special Populations
- Patients with hepatic impairment or Alzheimer's disease-related agitation should start with a dose of 2.5 mg, as recommended by the Praxis Medical Insights guidelines 1
- The American Academy of Emergency Medicine suggests that patients with Alzheimer's disease-related agitation may be treated with olanzapine, with a starting dose of 2.5 mg 2
Intramuscular Dosing
- Clinical trials demonstrated efficacy with IM doses ranging from 2.5-10 mg for acute agitation associated with schizophrenia or bipolar mania, as reported in the Annals of Emergency Medicine and Praxis Medical Insights guidelines 1, 2
Important Clinical Considerations
- The Praxis Medical Insights guidelines recommend using extreme caution when combining olanzapine with benzodiazepines due to risk of oversedation, respiratory depression, and reported fatalities 1
- Monitoring is required for drowsiness and sedation, as well as orthostatic hypotension, particularly with IM administration, as recommended by the Praxis Medical Insights guidelines 1
- Long-term use of olanzapine requires monitoring of metabolic effects, such as weight gain, glucose, and lipids, according to the Praxis Medical Insights guidelines 1
