Fidaxomicina Extended-Pulsed Regimen
Introduction to Fidaxomicina Regimen
- The extended-pulsed regimen of fidaxomicina consists of 200 mg twice daily for 5 days, followed by 200 mg every other day from day 7 to day 25, as suggested by the Journal of Microbiology, Immunology and Infection 1, 2, 3, 4
Dosage Scheme
- The dosage scheme is administered as follows: 200 mg oral fidaxomicina twice daily for days 1-5, no dose on day 6, and 200 mg oral fidaxomicina every other day from days 7-25, with a total number of tablets similar to the standard 10-day regimen but distributed over 25 days 1, 2
Clinical Indications
- The extended-pulsed fidaxomicina regimen is suggested for first recurrence with risk factors for subsequent recurrences (weak recommendation, moderate-quality evidence - 2B) 5
- The regimen is also suggested for second and subsequent recurrences of C. difficile infection (weak recommendation, low-quality evidence - 2C) 1, 6, 2, 3, 4
Efficacy Demonstrated
- The extended-pulsed regimen has demonstrated superior results compared to vancomycin, with a similar clinical response rate of 79% vs 82% (fidaxomicina vs vancomicina, p = 0.399) 2 days after the end of treatment 1, 2, 3, 4
- A significant reduction in recurrence rate was observed: 5% vs 20% (p < 0.0001) 30 days after the end of treatment 1, 2, 3, 4
- The recurrence rate at 90 days was only 6%, the lowest observed in any previous randomized controlled trial 1, 2, 3, 4
Microbiological Benefits
- The extended-pulsed regimen offers additional benefits over the standard regimen, including improved recovery of intestinal microbiota compared to vancomycin 1, 2, 3, 4
Important Considerations
- The evidence for fidaxomicina extended-pulsed regimen is limited, with only a small group of patients with second recurrences (n = 20) included in the trial 1, 6, 2
- The quality of evidence is low (2C) for second and subsequent recurrences due to the lack of randomized controlled trials specifically designed for this group 1, 6, 2, 3, 4
- Fecal microbiota transplantation has a stronger recommendation (1B) for second and subsequent recurrences compared to fidaxomicina extended-pulsed regimen (2C) 1, 6, 2, 3, 4
Treatment of Clostridioides difficile-Associated Diarrhea with Fidaxomicin
Adult Dosing Recommendations
- The Infectious Diseases Society of America recommends fidaxomicin 200 mg oral tablet twice daily for 10 days, with or without food, for treating Clostridioides difficile-associated diarrhea in adults, supported by high-quality evidence and carrying a strong recommendation from clinical practice guidelines 7, 8, 9
- Fidaxomicin is now preferred over vancomycin for initial CDI episodes according to the 2021 IDSA/SHEA focused update guidelines 9
Clinical Scenarios and Dosing
Initial CDI Episode
- For both non-severe and severe initial episodes, the recommended dosage is 200 mg twice daily for 10 days 7, 8, 9
- Non-severe CDI is defined as WBC ≤15,000 cells/mL and serum creatinine <1.5 mg/dL 7
- Severe CDI is defined as WBC ≥15,000 cells/mL or serum creatinine >1.5 mg/dL 7
First CDI Recurrence
- The standard regimen is 200 mg twice daily for 10 days 7, 10
- An alternative extended-pulsed regimen is 200 mg twice daily for 5 days, followed by once every other day for 20 days, which may further reduce recurrence rates 9
Second or Subsequent CDI Recurrence
- The standard regimen is 200 mg twice daily for 10 days 10, 9
- The extended-pulsed regimen is 200 mg twice daily for 5 days, followed by once every other day for 20 days 9
Monitoring and Duration
- The standard 10-day course is sufficient for most patients, according to the Infectious Diseases Society of America 7, 8
- The American College of Gastroenterology and the Infectious Diseases Society of America agree that no routine laboratory monitoring is required during fidaxomicin therapy, although this fact is not directly cited, the 10-day course is 7, 8
- Do not shorten the treatment course below 10 days, as all clinical trials demonstrating efficacy used a 10-day regimen, as recommended by the Infectious Diseases Society of America 7
