Citicoline in Acute Ischemic Stroke

Evidence Assessment

  • The American Heart Association/American Stroke Association guidelines do not recommend citicoline for the treatment of acute ischemic stroke, as multiple clinical trials have failed to demonstrate consistent efficacy in improving outcomes, with a strength of evidence based on multiple randomized controlled trials 1, 2
  • A patient-level pooled analysis suggested benefit in reducing disability when citicoline was started within 24 hours of symptom onset, although this finding is not supported by later definitive trials 3
  • The International Citicoline Trial on Acute Stroke (ICTUS), a large European multicenter randomized trial enrolling 2,298 patients with moderate to severe ischemic stroke, found no difference in the 90-day global outcome endpoint between citicoline and placebo (OR 1.03, 95% CI 0.86-1.25, p=0.364), providing high-quality evidence against its efficacy 1, 2

Practical Approach

  • For acute ischemic stroke management, focus on established evidence-based interventions, including rapid evaluation for thrombolysis with rtPA within the appropriate time window, as recommended by the American Heart Association/American Stroke Association 1
  • Early initiation of antiplatelet therapy (aspirin) within 24-48 hours of stroke onset in non-thrombolyzed patients is recommended, based on evidence from clinical trials 4
  • Management of physiological parameters, including blood pressure, is crucial in acute ischemic stroke care, with evidence supporting its importance in improving outcomes 3

Acute Ischemic Stroke Management

Evidence Assessment

  • The American Heart Association/American Stroke Association guidelines explicitly state that no agent with putative neuroprotective effects, including citicoline, can be recommended for the treatment of patients with acute ischemic stroke (Grade A recommendation) 5, 6
  • A post-hoc analysis found recovery at 3 months in 25.2% of citicoline-treated patients versus 20.2% of placebo-treated patients 6

Current Approach to Stroke Management

  • Admission to a stroke unit for specialized care and monitoring 7
