Anticoagulation Management

Standard INR Targets

  • The American College of Cardiology recommends a target INR of 2.0-3.0 for atrial fibrillation and most thromboembolic conditions requiring warfarin, aiming for a midpoint of 2.5 to maximize time in therapeutic range 1, 2, 3
  • For nonvalvular atrial fibrillation, the target INR is 2.0-3.0, as recommended by the American College of Cardiology and supported by the Journal of the American College of Cardiology 1, 2, 3, 4
  • For mechanical heart valves, the target INR is 2.0-3.0 or 2.5-3.5, depending on valve type and location, according to the Journal of the American College of Cardiology 4, 5

Critical INR Thresholds

  • An INR below 2.0 significantly increases the risk of thromboembolism and ischemic stroke, as reported by Chest 1
  • An INR above 3.0 increases the incidence of major bleeding, according to Chest 1
  • An INR above 3.5 markedly elevates the risk of intracranial hemorrhage, as stated by Chest 1

Time in Therapeutic Range (TTR) Requirements

  • Warfarin therapy requires a TTR of ≥65-70% to be considered adequately controlled, with a target of ≥70%, as recommended by Chest and the European Heart Journal 1, 2, 3, 7
  • If TTR falls below 65%, switching to DOACs or implementing intensive interventions is necessary, according to Chest 1, 2, 3

When Warfarin is Required Over DOACs

  • The American College of Cardiology mandates warfarin for all patients with mechanical heart valves, as DOACs are absolutely contraindicated, supported by the Journal of the American College of Cardiology and the European Heart Journal 4, 5, 7, 8
  • For moderate-to-severe mitral stenosis, warfarin is required, as DOACs are contraindicated, according to the European Heart Journal and the Journal of the American College of Cardiology 7, 8, 5

When Warfarin is Preferred Over DOACs

  • For severe renal dysfunction, with a creatinine clearance <15 mL/min or patients on hemodialysis, warfarin is preferred, with a target INR of 2.0-3.0, as recommended by the Journal of the American College of Cardiology 4
  • For left ventricular thrombus, warfarin with a target INR of 2.0-3.0 is preferred, according to Praxis Medical Insights 6

When DOACs are Preferred Over Warfarin

  • For all eligible patients with nonvalvular atrial fibrillation, DOACs are strongly recommended over warfarin, as stated by Chest, the European Heart Journal, and the Journal of the American College of Cardiology 1, 2, 7, 8, 5
  • For patients with prior unprovoked bleeding, warfarin-associated bleeding, or high bleeding risk, apixaban, edoxaban, or dabigatran 110 mg are preferred, according to Chest 1, 2, 3

Monitoring Requirements

  • Weekly INR monitoring is recommended during warfarin initiation, and monthly monitoring when stable, as suggested by the Journal of the American College of Cardiology 4, 8, 5
  • No routine laboratory monitoring is required for DOACs, but renal function monitoring is essential, particularly for dabigatran, according to Praxis Medical Insights 6
