Psychopharmacological Treatment for Anxiety and Obsessive-Compulsive Disorders

First-Line Pharmacological Agents

  • The American Psychiatric Association recommends selective serotonin reuptake inhibitors (SSRIs) as first-line treatment for anxiety disorders and obsessive-compulsive disorder, with sertraline and escitalopram being preferred choices due to their favorable safety profiles and efficacy 1, 2, 3, 4, 5, 6
  • SSRIs are recommended as first-line treatment for both anxiety disorders and obsessive-compulsive disorder, with similar effect sizes for treating these conditions 7, 1, 2, 3, 4, 5, 6
  • Sertraline is specifically recommended as a preferred option due to its favorable safety profile and low potential for drug interactions 8
  • Escitalopram is also preferred because it has the least effect on CYP450 isoenzymes compared to other SSRIs, resulting in lower propensity for drug interactions 8

Specific SSRI Dosing and Duration

  • For obsessive-compulsive disorder, use maximum recommended or tolerated doses of SSRIs for at least 8 weeks at this dose 7
  • For anxiety disorders, start sertraline at 25 mg daily and increase doses at 1-2 week intervals, monitoring for tolerability 8
  • Treatment should continue for at least 4-12 months after symptom remission for a first episode, with longer-term or indefinite treatment for recurrent anxiety 8

Second-Line Pharmacological Agents

  • Venlafaxine (SNRI) is suggested as an alternative first-line agent for anxiety disorders and can be recommended for generalized anxiety disorder 1, 2, 3, 4, 5, 6
  • SNRIs (venlafaxine or duloxetine) are appropriate alternatives if SSRIs are ineffective or not tolerated 8

Combination with Psychotherapy

  • Cognitive behavioral therapy (CBT) is the psychotherapy with the highest level of evidence for anxiety disorders and OCD 1, 2, 3, 4, 5, 8
  • A combination of medication and CBT is a clinically desired treatment strategy for optimal outcomes, although individual CBT sessions are generally preferred over group therapy due to superior clinical effectiveness 8
