Acne Treatment Guidelines

Introduction to Acne Treatment

  • The American Academy of Dermatology recommends starting with adapalene 0.1-0.3% gel or tretinoin 0.025-0.1% cream/gel once nightly plus benzoyl peroxide 2.5-5% once daily for mild acne, and adding doxycycline 100 mg once daily or minocycline 100 mg once daily for moderate-to-severe inflammatory acne 1, 2

Topical Retinoid Dosing

  • The American Academy of Dermatology recommends applying tretinoin 0.025%, 0.05%, or 0.1% cream or gel once nightly to completely dry skin, starting with lower concentrations in sensitive skin and titrating up as tolerated 1
  • Adapalene 0.1% or 0.3% gel can be applied once nightly, and is more photostable than tretinoin and can be used with benzoyl peroxide 1, 3

Benzoyl Peroxide Dosing

  • The American Academy of Dermatology recommends applying 2.5-5% gel, cream, or wash once or twice daily, and always combining with antibiotics to prevent bacterial resistance 1, 3

Topical Antibiotic Dosing

  • Clindamycin 1% gel, lotion, or solution can be applied twice daily, always combined with benzoyl peroxide, and fixed-combination products can enhance compliance 1, 3
  • Erythromycin 2-3% gel or solution can be applied twice daily, always combined with benzoyl peroxide, and fixed-combination products can be used twice daily 1, 3

Oral Antibiotic Dosing

  • Doxycycline 100 mg once or twice daily with food is recommended, with a maximum duration of 3-4 months to minimize resistance development 1, 3
  • Minocycline 100 mg once or twice daily is conditionally recommended, with a maximum duration of 3-4 months 1, 2, 3

Oral Isotretinoin Dosing

  • For severe acne, the American Academy of Dermatology recommends starting with 0.5 mg/kg/day and increasing to 1.0 mg/kg/day as tolerated, targeting a cumulative dose of 120-150 mg/kg 1, 2
  • For moderate treatment-resistant acne, 0.25-0.4 mg/kg/day can be used, with equal efficacy and fewer side effects compared to standard dosing 2

Hormonal Therapy Dosing

  • Combined oral contraceptives are conditionally recommended for acne with moderate certainty evidence, and should be used at standard contraceptive dosing per product labeling 1, 3
  • Spironolactone 25-200 mg daily can be used, particularly for hormonal acne patterns or antibiotic-intolerant patients, without the need for potassium monitoring in healthy patients 1, 3

Additional Topical Agents

  • Azelaic acid 15-20% gel or cream can be applied twice daily, particularly for post-inflammatory hyperpigmentation 1, 3
  • Clascoterone 1% cream can be applied twice daily, conditionally recommended due to high cost 1, 3

Maintenance Therapy Dosing

  • The American Academy of Dermatology recommends continuing topical retinoid once nightly indefinitely after clearance, and adding benzoyl peroxide 2.5-5% once daily if needed for more severe baseline disease 1, 3
