Levetiracetam Management Guidelines

Introduction to Levetiracetam

  • Levetiracetam is an effective antiepileptic medication with a favorable safety profile that can be administered at doses of 1000-3000 mg/day for most adults with seizure disorders, with dose adjustments based on renal function and clinical response 1, 2

Dosing and Administration

  • The initial dose of levetiracetam is 500 mg twice daily for adults 1
  • A loading dose of 1500 mg orally or up to 60 mg/kg IV can be used for rapid initiation 1
  • In emergency or status epilepticus situations, a loading dose of 30-50 mg/kg IV at a rate of 100 mg/min can be administered 2
  • Levetiracetam can be administered intravenously at rates up to 100 mg/min 2

Special Populations and Considerations

  • For pediatric dosing, an initial dose of 10 mg/kg twice daily can be used, with a maximum dose of 60 mg/kg/day in clinical trials 3, 5
  • In patients receiving CAR T-cell therapy who have CNS disease or history of seizures, levetiracetam is recommended for seizure prophylaxis at a dose of 10 mg/kg (maximum 500 mg per dose) every 12 hours for 30 days following infusion 3, 4

Efficacy and Monitoring

  • Levetiracetam has been shown to have a 67-73% seizure termination rate in refractory status epilepticus 5
  • The neurological status of patients should be monitored for changes, especially when used for status epilepticus 1

Adverse Effects

  • Common adverse effects of levetiracetam include somnolence and fatigue 1
