Intravenous Metoprolol Dosing for Patients Unable to Take Oral Medications

Standard IV Metoprolol Dosing Protocol

  • The American Heart Association recommends administering metoprolol tartrate as 5 mg boluses every 5 minutes as tolerated, up to a maximum of 3 doses (15 mg total), with careful titration to heart rate and blood pressure 1, 2

Clinical Indications for IV Metoprolol

  • IV metoprolol is indicated for refractory hypertension requiring rapid control, ongoing myocardial ischemia, and acute rate control in atrial fibrillation or flutter 1, 2, 3, 4

Contraindications and Precautions

  • The American College of Cardiology recommends avoiding IV metoprolol in patients with signs of heart failure, low output states, increased risk for cardiogenic shock, prolonged first-degree or high-grade AV block, and reactive airways disease 1, 2

Monitoring During IV Administration

  • The American Heart Association recommends frequent blood pressure checks and auscultation for rales and bronchospasm during IV metoprolol administration 5
  • Monitoring for signs of worsening heart failure is also recommended during IV metoprolol administration 4

Clinical Pearls

  • The onset of action of IV metoprolol is approximately 5 minutes after administration, as stated by the Journal of the American College of Cardiology 4, 6
  • The European Society of Cardiology notes that IV beta-blockers are contraindicated in patients with Wolff-Parkinson-White syndrome and tachycardia with ventricular pre-excitation 7
