Management of Osteomyelitis with Nondisplaced Intra-articular Fracture of Proximal Phalanx Great Toe

Diagnostic Approach

  • Obtain inflammatory markers to assess infection severity and establish baseline values for monitoring treatment response, as recommended by the Infectious Diseases Society of America 1, 3
  • Perform plain radiographs as first-line imaging to evaluate both the fracture and potential osteomyelitis, according to the Clinical Infectious Diseases guidelines 4
  • Consider MRI if diagnosis remains uncertain after initial radiographs, as it is the most accurate imaging study for defining bone infection and soft tissue involvement, based on the Clinical Infectious Diseases recommendations 4
  • Collect appropriate specimens for culture to identify causative microorganisms, including wound/sinus tract cultures and bone cultures, as suggested by the Radiology guidelines 3, 6, 7

Surgical Management

  • Urgent surgical consultation (within 24-48 hours) is recommended for moderate to severe infections to remove infected and necrotic tissue, according to the Diabetes/Metabolism Research and Reviews guidelines 2
  • Surgical intervention should address both the infection and the fracture, including debridement of all infected and necrotic bone and soft tissue, and fracture stabilization as appropriate, based on the Clinical Infectious Diseases recommendations 2, 4

Antibiotic Therapy

  • Start empiric antibiotic therapy targeting likely pathogens after obtaining cultures, as recommended by the Clinical Infectious Diseases guidelines 8, 1
  • For mild to moderate infections without recent antibiotic exposure, initial therapy aimed at aerobic gram-positive cocci may be sufficient, according to the Clinical Infectious Diseases guidelines 8
  • For severe infections, use broad-spectrum empiric therapy pending culture results, as suggested by the Clinical Infectious Diseases and Diabetes/Metabolism Research and Reviews guidelines 8, 1
  • Adjust definitive therapy based on culture results and clinical response, as recommended by the Clinical Infectious Diseases and Diabetes/Metabolism Research and Reviews guidelines 8, 1

Monitoring and Follow-up

  • Patients require early and careful follow-up to ensure that selected medical and surgical treatment regimens are effective, as recommended by the Clinical Infectious Diseases guidelines 8
  • Monitor inflammatory markers to assess treatment response, as suggested by the Diabetes/Metabolism Research and Reviews and Praxis Medical Insights guidelines 1, 5
  • If infection fails to respond to initial antibiotic course, consider discontinuing all antimicrobials for a few days, obtaining new culture specimens, and reassessing need for additional surgical intervention, according to the Clinical Infectious Diseases and Diabetes/Metabolism Research and Reviews guidelines 8, 2
