Management of Severe Digoxin Toxicity
Assessment and Diagnosis
- A digoxin level of 3.8 ng/mL is significantly above the therapeutic range of 0.5-0.9 ng/mL and indicates severe toxicity requiring immediate intervention 1
- Overt digoxin toxicity is commonly associated with serum levels >2.0 ng/mL, making patients at high risk for life-threatening complications 2
- Clinical manifestations of toxicity include cardiac arrhythmias, gastrointestinal symptoms, and neurological complaints, which are associated with serum levels >2.0 ng/mL 2
Treatment and Management
- The American Heart Association, as published in Circulation, recommends that patients with severe digoxin toxicity be treated with digoxin-specific antibody fragments (Digibind) 2
- When resuming digoxin therapy after toxicity resolves, the American College of Cardiology recommends using a lower dose, with a target serum concentration of 0.5-0.9 ng/mL 1
- Regular monitoring of digoxin levels, renal function, and electrolytes is recommended when restarting digoxin therapy, according to the American Heart Association 3
