Metronidazole Treatment for Amoebic Liver Abscess

Dosing Regimen

  • The standard oral dose of metronidazole for amoebic liver abscess is 500 mg three times daily for 7-10 days, achieving cure rates over 90% in uncomplicated cases 1
  • This dosing regimen is the preferred treatment, with most patients responding within 72-96 hours of initiating therapy 1

Critical Management Considerations

  • If pyogenic abscess cannot be excluded, broad-spectrum antibiotics (e.g., ceftriaxone plus metronidazole) should be added until amoebic serology confirms diagnosis 1
  • All patients must receive a luminal amoebicide after completing metronidazole to prevent relapse, even with negative stool microscopy 1
  • Diloxanide furoate 500 mg orally three times daily for 10 days is recommended as a luminal amoebicide 1

Important Safety Warnings

  • Prolonged courses of metronidazole beyond 14 days should be avoided due to cumulative and potentially irreversible neurotoxicity 2
  • Patients should be monitored closely for neurological symptoms during treatment 2
