Treatment of Carpal Tunnel Pain
First-Line Treatment Options
- The American Academy of Physical Medicine and Rehabilitation recommends oral corticosteroids are not specifically recommended in guidelines for carpal tunnel syndrome treatment 1, 2
Medication Considerations
- Systemic steroids, such as prednisone, carry potential adverse effects including hyperglycemia, hypertension, fluid retention, mood changes, and increased risk of infection in patients with carpal tunnel syndrome 1, 3
- Gabapentin's common side effects include dizziness, somnolence, and peripheral edema in patients with carpal tunnel syndrome 5
- The Mayo Clinic recommends gabapentin starting dose is typically 100-300 mg at bedtime or 100-300 mg three times daily, with gradual titration as tolerated in patients with carpal tunnel syndrome 3, 4
- Gabapentin requires gradual titration and may take 3-8 weeks plus 2 weeks at maximum dose for an adequate trial in patients with carpal tunnel syndrome 3, 4
- Gabapentin dosage should be reduced in patients with impaired renal function and carpal tunnel syndrome 3
