Typhoid Fever Treatment with Levofloxacin

Introduction to Levofloxacin Dosing

  • The American Academy of Pediatrics recommends that levofloxacin is generally not used for routine pediatric use due to concerns about effects on bone and cartilage growth, however, when benefits outweigh risks in multidrug-resistant typhoid, children ≥5 years can be given 10 mg/kg once daily (maximum 750 mg) 1, 2

Pediatric Considerations

  • For children <5 years, 10 mg/kg divided every 12 hours is recommended due to faster drug clearance 2

Critical Clinical Context

  • The Centers for Disease Control and Prevention advises exercising caution when treating travelers returning from India, as fluoroquinolone-resistant Salmonella typhi strains with decreased susceptibility are increasingly common 1

Important Contraindications

  • The American College of Obstetricians and Gynecologists recommends avoiding levofloxacin in pregnancy due to teratogenic effects, and alternative agents must be selected for pregnant patients with typhoid fever 2, 3

Renal Dose Adjustments

  • The National Kidney Foundation recommends that for creatinine clearance <50 mL/min, the dose should be reduced to 750 mg three times weekly or adjusted based on specific clearance values, as levofloxacin is 80% renally cleared and requires dose modification to prevent accumulation and toxicity 2

Common Pitfalls to Avoid

  • The Infectious Diseases Society of America recommends not administering levofloxacin within 2 hours of antacids or medications containing divalent cations (calcium, magnesium, aluminum, iron), as these markedly decrease fluoroquinolone absorption 2
  • The American College of Clinical Pharmacy advises not using 500 mg when 750 mg is indicated, as suboptimal peak concentrations may lead to treatment failure, particularly with concentration-dependent killing 3
