Treatment of Retrograde Ejaculation

First-Line Treatment

  • The American Urological Association recommends first-line treatment with sympathomimetic medications, such as pseudoephedrine 60-120 mg taken 120-150 minutes prior to ejaculation or imipramine 25-75 mg daily, for men with retrograde ejaculation seeking fertility, with pseudoephedrine showing the highest success rates for converting retrograde to antegrade ejaculation 1, 2
  • Pseudoephedrine is the most effective single agent, with a dosing regimen of 60-120 mg taken 120-150 minutes prior to sexual activity 1, 2

Adjunctive Measures for Fertility

  • Urine alkalinization to preserve sperm viability can be achieved by performing urethral catheterization after ejaculation to retrieve sperm from the bladder 1, 2

Assisted Reproductive Techniques

  • Assisted reproductive techniques, such as penile vibratory stimulation, electroejaculation, and surgical sperm retrieval, can be used when medical therapy fails 1, 2

Medication-Induced Retrograde Ejaculation

  • In cases of medication-induced retrograde ejaculation, consideration should be given to dose reduction or medication substitution before adding sympathomimetics 3

Treatment of Retrograde Ejaculation

  • Alpha-1 blockers, particularly tamsulosin and silodosin, commonly cause ejaculatory dysfunction including retrograde ejaculation, according to the European Urology 4
