Nitrates and Tadalafil Contraindication

Introduction to Contraindication

  • The American College of Cardiology recommends that patients who have taken tadalafil cannot receive any form of nitrate therapy for at least 48 hours after the last dose, due to the risk of profound hypotension, myocardial infarction, and death 1, 2, 3, 4

Mechanism of Interaction

  • Tadalafil inhibits phosphodiesterase-5 (PDE5), which degrades cyclic guanosine monophosphate (cGMP), and when combined with nitrates, causes markedly exaggerated and prolonged vasodilation, leading to profound hypotension, myocardial infarction, and potentially death 5, 6, 7

Clinical Consequences

  • The ACC/AHA guidelines state that nitrate use within 48 hours of tadalafil has been associated with profound hypotension, MI, and even death, and is a Class III recommendation (harm), meaning the intervention should not be performed 1, 2, 3, 5, 6, 7

Alternative Therapies

  • The American College of Cardiology suggests considering beta-blockers, calcium channel blockers, or ACE inhibitors as alternative anti-ischemic options when nitrates are contraindicated 2, 3
