Amoxicillin Dosing for Acute Otitis Media

Introduction to Treatment

  • The American Academy of Pediatrics recommends prescribing amoxicillin 80-90 mg/kg/day divided into two doses daily for a child with acute otitis media, which equals approximately 1400-1550 mg total daily dose, or 700-775 mg twice daily for 5 days, for a 3-year-old child weighing 38 pounds (17.3 kg) 1, 2

Weight-Based Calculation

  • For a child weighing 38 pounds (17.3 kg), the total daily dose of amoxicillin is 80-90 mg/kg/day, which equals 1384-1557 mg/day, divided into two doses of approximately 700-780 mg twice daily 1, 2

Treatment Duration

  • For children over 2 years of age, 5 days of treatment with amoxicillin is appropriate, as recommended by the American Academy of Pediatrics and the European Society of Clinical Microbiology and Infectious Diseases 1, 3

Why High-Dose Amoxicillin

  • High-dose amoxicillin (80-90 mg/kg/day) is recommended as first-line therapy because it achieves middle ear fluid levels that exceed the minimum inhibitory concentration for intermediately resistant and many highly resistant Streptococcus pneumoniae strains, according to the American Academy of Pediatrics 1

When to Consider Alternative Antibiotics

  • The American Academy of Pediatrics recommends switching to amoxicillin-clavulanate (90 mg/kg/day of amoxicillin component with 6.4 mg/kg/day clavulanate in 2 divided doses) if the child has taken amoxicillin within the past 30 days, or if concurrent purulent conjunctivitis is present, or if treatment failure occurs after 48-72 hours 1, 2, 3, 5

Monitoring for Treatment Failure

  • The European Society of Clinical Microbiology and Infectious Diseases recommends reassessing the child at 48-72 hours if symptoms persist or worsen, and defines treatment failure as worsening of the patient's condition, persistence of symptoms beyond 48 hours of antibiotic therapy, or recurrence of symptoms within 4 days after treatment completion 1, 2, 3

Common Pitfalls to Avoid

  • The American Academy of Pediatrics advises against using standard-dose amoxicillin (40 mg/kg/day) in areas with drug-resistant S. pneumoniae, and recommends ensuring adequate visualization of the tympanic membrane before prescribing antibiotics 1, 3, 5

Special Considerations for This Age Group

  • For children over 2 years of age, observation without immediate antibiotics is reasonable for non-severe cases, but this requires certain criteria, including absence of high fever, absence of intense earache, and reliable follow-up within 48-72 hours, as recommended by the European Society of Clinical Microbiology and Infectious Diseases 3, 4, 5
