Management of Gallbladder Polyps

Risk Stratification Based on Polyp Characteristics

  • For a 15-year-old with a gallbladder polyp, management should be based primarily on polyp size, with surgical consultation recommended only if the polyp is 15 mm or larger, or if it demonstrates rapid growth of 4 mm or more within a 12-month period 1, 2
  • Polyps 10-14 mm require periodic follow-up ultrasound at 6, 12, and 24 months 1, 3
  • Polyps 15 mm or larger warrant surgical consultation regardless of other factors 1
  • Pedunculated polyps with thin stalks have lower risk and can be classified as extremely low risk 4, 5
  • Sessile polyps should be considered higher risk 1, 4

Follow-Up Protocol

  • For polyps 10-14 mm, more frequent monitoring with ultrasound at 6, 12, and 24 months is recommended 1, 5
  • If the polyp grows to 15 mm or larger, surgical consultation is recommended 1, 2
  • If the polyp grows by 4 mm or more within a 12-month period, surgical consultation is recommended regardless of absolute size 1, 3
  • Growth of up to 3 mm may be part of the natural history of benign gallbladder polyps and should not necessarily trigger surgical intervention 1, 3

Special Considerations

  • Primary sclerosing cholangitis significantly increases malignancy risk and requires different management protocols than standard polyps 7, 8
