Management of MCA Territory Infarct in Young Females

Initial Assessment and Imaging

  • The American Heart Association recommends performing rapid neurological assessment to identify stroke symptoms and determine eligibility for reperfusion therapies, and obtaining immediate neuroimaging with either non-contrast CT or MRI with diffusion-weighted imaging (DWI) to detect acute ischemia 1, 4
  • Vascular imaging, such as CT angiography (CTA) or MR angiography (MRA), should be performed during initial evaluation to identify vessel occlusion 1

Predictors of Malignant Course

  • Clinical predictors of malignant MCA infarction include high stroke severity scores, nausea/vomiting, and progressive neurological deterioration, as well as history of hypertension, heart failure, and elevated white blood cell count 3, 6
  • Radiological predictors include early CT hypodensity, presence of hyperdense MCA sign, involvement of additional vascular territories, and large hypoperfusion on CT perfusion maps 6

Management of Brain Edema

  • The American College of Cardiology recommends initial management of brain swelling, including restriction of free water, correction of factors that exacerbate swelling, elevation of head of bed, and avoidance of antihypertensive agents that cause cerebral vasodilation 6, 7
  • Osmotic therapy with target serum osmolarity of 315-320 mOsm/L and brief moderate hyperventilation can be used as a bridge to more definitive therapy 3

Surgical Intervention

  • Decompressive hemicraniectomy should be considered in patients with malignant MCA infarction, with optimal timing within 48 hours of stroke onset, and reduces mortality by approximately 50% in patients ≤60 years with unilateral MCA infarction 2, 3

General Management

  • The American Heart Association recommends comprehensive evaluation for stroke etiology in young patients, including cardiac evaluation, carotid and vertebral artery assessment, and evaluation for hypercoagulable states 5
  • Decisions related to DNR orders or palliative care should be deferred for 24-48 hours after stroke onset to allow time to assess response to therapy 2
