Folinic Acid Dosing Guidelines
Introduction to Folinic Acid Therapy
- The National Comprehensive Cancer Network recommends leucovorin dosing at 400 mg/m² IV over 2 hours on day 1 for patients receiving methotrexate as part of chemotherapy regimens 1, 2
- The American Academy of Pediatrics recommends folinic acid supplementation at 10-20 mg three times weekly for prevention of methotrexate toxicity 3
Dosing Considerations
- For treatment of folate deficiency, the Clinical Nutrition society recommends 1-5 mg of folinic acid daily 5
- For patients on chronic hemodialysis, the Clinical Nutrition society recommends 1-5 mg daily or 1 mg after each dialysis session to maintain therapeutic folate levels 5
- For children with toxoplasmosis, the American Academy of Pediatrics recommends 10-20 mg three times weekly when used alongside pyrimethamine 3
Weight-Based Dosing
- For a 91-pound patient, the dose would be approximately 520 mg (400 mg/m² × 1.3 m²) for chemotherapy protocols 1, 6
- For supplementation with methotrexate therapy, the standard 10-20 mg three times weekly is appropriate regardless of weight 3
Administration and Monitoring
- The Clinical Nutrition society prefers oral administration for long-term supplementation 5
- The National Comprehensive Cancer Network recommends intravenous administration for chemotherapy protocols 1, 2
- Monitoring of complete blood count, liver function, and renal function is recommended regularly during therapy 4
- The Clinical Nutrition society recommends measuring folate status in plasma or serum (for short-term status) or RBC (for long-term status) using validated methods 5
