Administration of Albumin and Furosemide in Clinical Practice

Patient Management Recommendations

  • For patients requiring both albumin and furosemide, such as those with cirrhosis and ascites, the American Association for the Study of Liver Diseases recommends administering these medications separately, using different infusion sites 1, 2
  • When sequential administration is necessary, the European Association for the Study of the Liver suggests completing the albumin infusion first, followed by furosemide administration afterward 2
  • In patients with cirrhosis and large volume ascites requiring paracentesis, the European Association for the Study of the Liver recommends albumin as plasma volume expansion after the procedure, with diuretics administered separately 3, 4
  • The American Gastroenterological Association recommends monitoring electrolytes, kidney function, and hemodynamic parameters closely when using both medications, even when administered separately 5

Special Considerations

  • In spontaneous bacterial peritonitis, the American Gastroenterological Association recommends albumin in addition to antibiotics, but furosemide should be administered separately if needed 2
  • The European Association for the Study of the Liver advises discontinueing diuretics if severe hyponatremia, acute kidney injury, worsening hepatic encephalopathy, or incapacitating muscle cramps develop 3, 4
