Resistance Training for Hypertension Management

Benefits of Resistance Training

  • The International Society of Hypertension recommends strength training as part of their lifestyle modifications for hypertension management, with a frequency of 2-3 days per week 1, 2
  • Perform resistance/strength exercises on 2-3 days per week, focusing on dynamic resistance training rather than isometric exercises 1, 2, 3
  • Include 8-10 different exercises targeting major muscle groups 4
  • Maintain proper breathing technique during exercises to avoid excessive blood pressure spikes 5

Combining with Other Lifestyle Modifications

  • Combine resistance training with aerobic exercise (30 minutes on 5-7 days per week) for optimal blood pressure management 2, 3
  • Combine resistance training with dietary modifications (reduced sodium, increased potassium) for optimal blood pressure management 2, 6
  • Combine resistance training with weight management for those who are overweight, and moderation of alcohol consumption 2, 5, 7

Special Considerations

  • For individuals with severe hypertension (≥160/100 mmHg), blood pressure should be controlled before initiating an intensive resistance training program 8
  • Start with lighter weights and gradually increase intensity as tolerance improves 4
  • Monitor blood pressure response to exercise, especially when beginning a new program 9
