Treatment of Vitamin B12 Deficiency

Initial Treatment Protocol

  • The American Academy of Pediatrics recommends hydroxocobalamin 1 mg intramuscularly three times a week for 2 weeks for patients without neurological involvement 1
  • For patients with neurological involvement, hydroxocobalamin 1 mg intramuscularly on alternate days is recommended until there is no further improvement 1, 2
  • After initial treatment, maintenance therapy with hydroxocobalamin 1 mg intramuscularly every 2-3 months for life is recommended 1, 2

Treatment Based on Cause of Deficiency

  • The British Medical Journal suggests determining the underlying cause of B12 deficiency, which could include a diet low in vitamin B12, malabsorption issues, or medication effects 3, 4
  • For deficiency due to malabsorption, parenteral vitamin B12 will be required for life 1

Dosage Considerations

  • The recommended dose of hydroxocobalamin is 1 mg, which is preferred over cyanocobalamin, especially in patients with renal dysfunction 1, 2
  • Hydroxocobalamin is recommended over cyanocobalamin for better retention 1

Monitoring and Follow-up

  • Checking both vitamin B12 and folate levels is recommended, as folate deficiency may coexist 1
  • Measuring methylmalonic acid can be considered as a confirmatory test when initial B12 results are indeterminate 1
