Management of Oxcarbazepine Drug Reactions
Immediate Assessment and Discontinuation
- Assess airway, breathing, and circulation following standard resuscitation protocols, and monitor vital signs, cardiac rhythm, and neurological status continuously in patients with suspected oxcarbazepine hypersensitivity reactions 1
- Establish intravenous access for medication administration in patients with suspected oxcarbazepine hypersensitivity reactions 2
Type of Reaction Determines Management Approach
Serious Dermatological Reactions (SJS/TEN/DRESS)
- Administer corticosteroids at an equivalent dose of 1-2 mg/kg of IV methylprednisolone every 6 hours for severe reactions, as recommended by the American Society of Clinical Oncology 3, 4
Anaphylaxis or Angioedema
- Administer epinephrine 0.2-0.5 mg (1 mg/mL) intramuscularly into the lateral thigh, repeat every 5-15 minutes as needed, and provide aggressive fluid resuscitation with normal saline 1-2 L IV at 5-10 mL/kg in first 5 minutes, as recommended by the American Heart Association 3, 4
- Give H1/H2 antagonists: diphenhydramine 50 mg IV plus ranitidine 50 mg IV (or cimetidine 300 mg IV), and corticosteroids equivalent to 1-2 mg/kg IV methylprednisolone every 6 hours, as recommended by the American Academy of Allergy, Asthma, and Immunology 3, 4
Overdose Management
- Secure airway with bag-mask ventilation or endotracheal intubation if CNS depression with loss of protective reflexes occurs, and administer benzodiazepines (diazepam first-line or midazolam) for seizures or severe agitation, as recommended by the American Association of Poison Control Centers 2
- Contact poison control (1-800-222-1222 in US) for expert guidance, and monitor ECG for intraventricular conduction delay, as recommended by the American Heart Association 2
