Intertrochanteric Femur Fracture Fixation with Dynamic Hip Screw (DHS) Plating
Preoperative Preparation
- The standard surgical treatment for intertrochanteric femur fractures is the Dynamic Hip Screw (DHS), allowing for stable fixation and early mobilization to reduce morbidity and mortality in patients with this type of fracture 1
- Administer prophylactic antibiotics within one hour of skin incision to reduce infection risk in patients undergoing DHS plating 2
- Ensure appropriate anesthesia, such as spinal/epidural or general anesthesia with consideration of peripheral nerve blocks for postoperative pain management, in patients undergoing intertrochanteric femur fracture fixation with DHS plating 3, 2
- Implement active warming strategies to prevent hypothermia, which is particularly important in elderly patients undergoing intertrochanteric femur fracture fixation with DHS plating 2
Postoperative Management
- Continue appropriate thromboprophylaxis, such as fondaparinux or low molecular weight heparin, in patients after intertrochanteric femur fracture fixation with DHS plating 2
- Implement early mobilization protocols to reduce complications in patients after intertrochanteric femur fracture fixation with DHS plating 1
- Provide adequate pain management with regular paracetamol and cautious use of opioids, especially in patients with renal dysfunction after intertrochanteric femur fracture fixation with DHS plating 2
