Guideline Recommendations for Managing ASCA IgG‑Positive Crohn’s Disease

Diagnostic Evaluation

  • Perform ileocolonoscopy with biopsies to confirm active inflammation, define disease location and extent (especially small‑bowel involvement), and obtain histologic confirmation of Crohn’s disease – recommended by the Gastroenterology 2023 consensus statements. 1, 2, 3

  • Obtain baseline inflammatory biomarkers (fecal calprotectin and serum C‑reactive protein) to aid in initial disease assessment and to serve as reference values for future monitoring – supported by the Gastroenterology 2023 consensus statements. 1, 2, 3

Ongoing Monitoring

  • After diagnosis, use fecal calprotectin and C‑reactive protein for routine disease activity monitoring rather than repeating ASCA testing, in accordance with American Gastroenterological Association (AGA) guidelines – based on the Gastroenterology 2023 consensus statements. 1, 2, 3
