Dosing Equivalency for Olanzapine and Quetiapine

General Guidelines

  • The American College of Clinical Pharmacology recommends that when switching between antipsicotics, a gradual transition should be considered to minimize the risk of psychotic relapse or withdrawal symptoms, with olanzapina requiring a dose of 7,5-10 mg/día with gradual increases according to clinical response 1
  • The National Comprehensive Cancer Network suggests that olanzapina should be used with caution in elderly patients due to the higher risk of adverse effects, including somnolence, fatigue, and sleep disturbances 2
  • The National Comprehensive Cancer Network also advises avoiding excessive dopamine blockade when using olanzapina with metoclopramide, phenothiazines, or haloperidol 3

Special Population Considerations

  • For elderly patients or those at risk of excessive sedation, a lower dose of olanzapina (5 mg) may be considered 3
  • Both medications require monitoring for metabolic side effects, although with different risk profiles 1
