Diagnostic Evaluation and Management of Myelodysplastic/Myeloproliferative Neoplasms
Diagnostic Criteria and Evaluation
- The National Comprehensive Cancer Network recommends a comprehensive diagnostic workup, including complete blood count with differential and peripheral blood smear examination, bone marrow aspiration and biopsy with iron stain, cytogenetic analysis, and serum erythropoietin level, for patients with suspected myelodysplastic/myeloproliferative neoplasms 1
- Evaluation for PDGFRα/β gene rearrangements is essential, especially in patients with eosinophilia, as recommended by the Haematologica guidelines 2, 3
- JAK2 mutation testing is recommended in patients with thrombocytosis, according to the Journal of the National Comprehensive Cancer Network 3, 4
Classification and Risk Stratification
- The Haematologica guidelines classify MDS/MPN into subtypes, including myelodysplastic-type CMML (MD-CMML) and myeloproliferative-type CMML (MP-CMML), based on WBC count and blast percentage 2, 5
- Risk assessment should incorporate blast percentage, cytogenetic abnormalities, number of cytopenias, and age, as recommended by the Journal of the National Comprehensive Cancer Network 6, 7
Initial Treatment Approach
- The Haematologica guidelines recommend supportive care, including red blood cell transfusions and platelet transfusions, for patients with myelodysplastic-type CMML (MD-CMML) and <10% bone marrow blasts 8
- For patients with myeloproliferative-type CMML (MP-CMML) and <10% bone marrow blasts, cytoreductive therapy with hydroxyurea is recommended, according to the Haematologica guidelines 8
- The Journal of the National Comprehensive Cancer Network recommends imatinib mesylate as the treatment of choice for patients with PDGFRβ gene rearrangements 3
- For patients with JAK2 mutations and significant thrombocytosis, JAK2 inhibitors such as ruxolitinib may be considered, as recommended by the Praxis Medical Insights guidelines 4
Monitoring and Response Assessment
- Regular monitoring of blood counts is essential for all patients, and response should be evaluated using the International Consortium Proposal of response criteria for MDS/MPNs, although the specific criteria are not provided in the cited references 4
