Considerations for Prescribing Augmentin to Elderly Patients

Renal Function Assessment

  • The American Geriatrics Society recommends calculating creatinine clearance using the Cockcroft-Gault formula before prescribing Augmentin to elderly patients, as serum creatinine alone significantly underestimates renal impairment due to decreased muscle mass, and adjust dosing accordingly to prevent drug accumulation and toxicity in patients with reduced renal function 1, 2
  • Renal function may have declined by 40% by age 70, yet serum creatinine can remain within normal range in patients with actual renal impairment, emphasizing the need for accurate assessment using the Cockcroft-Gault formula 2

Dosing Adjustments and Nephrotoxicity

  • The National Kidney Foundation suggests avoiding co-prescription of NSAIDs or COX-2 inhibitors with Augmentin in elderly patients, as these are nephrotoxic and will worsen renal function, and preference should be given to agents less likely to be toxic to the kidneys or influenced by renal clearance 1, 2

Monitoring and Common Pitfalls

  • The European Renal Association recommends monitoring renal function regularly during Augmentin therapy to detect drug accumulation and prevent toxicity, particularly in elderly patients where renal function can deteriorate rapidly, and avoiding prolonged courses without reassessing renal function 3
