Clopidogrel Duration After Coronary Stent Implantation

Drug‑Eluting Stents (DES)

  • Patients receiving drug‑eluting stents should be prescribed clopidogrel 75 mg daily for a minimum of 12 months, regardless of whether the indication is acute coronary syndrome or stable coronary disease (Class I, Level B). 1, 2, 3, 4, 5
  • The 12‑month minimum applies to all contemporary DES platforms, including sirolimus‑eluting and paclitaxel‑eluting stents. (Class I, Level B). 1, 2, 6
  • Extending clopidogrel beyond 12 months may be considered in selected patients (Class IIb, Level C). 3, 7

Bare‑Metal Stents (BMS)

  • After BMS implantation, clopidogrel should be continued for at least 1 month. (Class I, Level B). 1, 2, 8, 4, 5
  • Ideally, therapy should be extended up to 12 months unless the patient has a high bleeding risk; in such cases a minimum of 2 weeks is acceptable before switching to aspirin monotherapy. (Class I, Level B). 1, 2, 8, 4, 5

Acute Coronary Syndrome (ACS) Context

  • All ACS patients receiving any stent (DES or BMS) should receive clopidogrel for ≥12 months, with alternative options of prasugrel 10 mg daily or ticagrelor 90 mg twice daily (Class I, Level B). 4, 5

Non‑ACS (Stable Coronary Disease) Context

  • For stable coronary disease treated with DES, the 12‑month clopidogrel duration remains the standard (Class I, Level B). 4, 5
  • For stable disease treated with BMS, a minimum of 1 month is required, with an ideal target of 12 months when bleeding risk permits (Class I, Level B). 4, 5

Bleeding‑Risk Modifications

  • When bleeding risk outweighs thrombotic benefit, earlier discontinuation (<12 months) is reasonable (Class IIa, Level C). 4, 5
  • In high‑bleeding‑risk patients with BMS, a minimum of 2 weeks of clopidogrel is acceptable before transitioning to aspirin alone. (Class I, Level B). 1, 2, 8, 4

Aspirin Co‑Administration

  • Aspirin must be continued indefinitely alongside clopidogrel during dual antiplatelet therapy (DAPT). (Class I, Level B). 1, 2, 6, 3
  • Initial aspirin dosing:
    • 162–325 mg daily for 1 month after BMS,
    • 162–325 mg daily for 3 months after sirolimus‑eluting DES,
    • 162–325 mg daily for 6 months after paclitaxel‑eluting DES,
  • After completing the prescribed clopidogrel course, aspirin monotherapy (75–162 mg daily) should be continued indefinitely. (Class I, Level B). 1, 2, 6

Evidence Supporting Prolonged DAPT (>12 months)

Outcome Effect of Prolonged DAPT (>12 mo) Evidence
Stent thrombosis Reduced (≈3 fewer events per 1,000 patients per year) [12]
Myocardial infarction Reduced (≈6 fewer events per 1,000 patients per year) [12]
Major bleeding Increased (≈5 more events per 1,000 patients per year) [12]
Overall odds ratio for major bleeding 1.58 (95 % CI 1.20–2.09) [12]
  • A 2016 ACC/AHA systematic review concluded that extending DAPT beyond 12 months decreases stent thrombosis and myocardial infarction but increases major bleeding (OR 1.58, 95 % CI 1.20‑2.09). (Class IIa, Level B). 12
