Management of Alcoholic Liver Disease with Upper GI Bleeding
Immediate Diagnostic Assessment
- The American Association for the Study of Liver Diseases recommends calculating severity scores, such as the Maddrey Discriminant Function (MDF) and MELD score, to assess the severity of alcoholic hepatitis and predict mortality 1, 5
- A patient's presentation with hematemesis, melena, jaundice, coagulopathy, thrombocytopenia, anemia, and hyperbilirubinemia is consistent with severe alcoholic hepatitis complicated by portal hypertension and variceal bleeding 2, 3, 4
Acute Bleeding Management
- The European Association for the Study of the Liver (EASL) guidelines state that correcting abnormal coagulation tests with blood products to prevent bleeding is not recommended and lacks evidence 6, 7
- Vasoactive drugs, such as octreotide or terlipressin, should be started immediately upon suspicion of variceal bleeding, before endoscopy 1
- Endoscopic therapy, such as band ligation, is preferred for esophageal varices, while sclerotherapy or cyanoacrylate is preferred for gastric varices 1
Infection Surveillance and Management
- Infection is the most critical factor to assess before any specific alcoholic hepatitis therapy, and empiric antibiotics should be started immediately if spontaneous bacterial peritonitis (SBP) is suspected 1
- The presence of infection, such as SBP, pneumonia, or UTI, is a contraindication to corticosteroid therapy 1
Corticosteroid Therapy Decision
- If the MDF is ≥32 or hepatic encephalopathy is present, and infection is definitively excluded, consider prednisolone 40 mg daily for 28 days 5, 4
Nutritional and Supportive Care
- Aggressive nutritional support is essential and improves outcomes, with a recommended intake of 1.0-1.5 g protein/kg/day and 30-40 kcal/kg/day 1
- Thiamine supplementation should be administered immediately to prevent Wernicke's encephalopathy 1
Disposition and Follow-up
- Hospital admission is required for severe alcoholic hepatitis with GI bleeding, coagulopathy, and high bilirubin, and early transplant evaluation should be considered if the MELD score is >26 or the patient fails to respond to medical therapy within 7 days 1
- Abstinence counseling should be initiated immediately, as abstinence is the single most effective intervention for long-term survival 5
