Ondansetron ODT Dosage for Adult Nausea Management

Dosing Guidelines

  • The American Society of Clinical Oncology recommends ondansetron ODT 8 mg orally, which can be administered once daily to twice daily as needed for breakthrough nausea and vomiting 1, 2
  • Ondansetron ODT 8 mg is the standard dose for adults experiencing nausea, allowing for convenient administration, particularly beneficial for patients who have difficulty swallowing conventional tablets 1, 2

Special Considerations

Chemotherapy-Induced Nausea

  • For chemotherapy with low to moderate emetogenic potential, the National Comprehensive Cancer Network recommends 8 mg ODT before chemotherapy, followed by 8 mg every 8 hours as needed 4
  • For highly emetogenic chemotherapy, the American Society of Clinical Oncology recommends 8 mg ODT before chemotherapy, with additional doses scheduled rather than as needed 2, 5

Radiation-Induced Nausea

  • For radiation to areas with low emetogenic risk, the American Society for Radiation Oncology recommends 8 mg ODT used as breakthrough therapy 6, 3
  • For radiation to areas with moderate to high emetogenic risk, the National Comprehensive Cancer Network recommends 8 mg ODT once to twice daily on treatment days, administered before radiation 1, 4

Treatment Algorithm

  • Initial treatment: Ondansetron ODT 8 mg at onset of nausea, as recommended by the American Society of Clinical Oncology 1, 2
  • If inadequate response after 1 hour: May repeat 8 mg dose, according to the American Society of Clinical Oncology 3
  • For persistent nausea despite maximum ondansetron dosing: Consider adding a different class of antiemetic, such as a dopamine antagonist, as suggested by the National Comprehensive Cancer Network 4, 3
