Metformin Extended-Release Dosing in Impaired Renal Function

Initial Dosing and Titration

  • The American College of Physicians recommends starting with 500 mg of metformin extended-release daily and titrating upward by 500 mg weekly until reaching the maximum dose appropriate for the patient's kidney function, with dose adjustments required when eGFR falls below 45 mL/min/1.73m² 1
  • Titrate upward by 500 mg increments every 7 days based on glycemic control and tolerability, as recommended by the Kidney International guidelines 1, 2

Dose Adjustments Based on Kidney Function

  • For patients with eGFR ≥60 mL/min/1.73m², the Kidney International guidelines recommend continuing standard dosing without adjustment, with a maximum dose of up to 2000-2250 mg daily, and monitoring kidney function at least annually 1, 3
  • For patients with eGFR 45-59 mL/min/1.73m², consider dose reduction in certain high-risk conditions, including advanced age, concomitant liver disease, or heart failure, with a maximum recommended dose of 1700 mg daily, as recommended by the Kidney International guidelines 1, 3
  • For patients with eGFR 30-44 mL/min/1.73m², reduce dose to half the maximum, typically 500-1000 mg daily, and monitor kidney function every 3-6 months, as recommended by the Kidney International guidelines 1, 2, 3
  • For patients with eGFR <30 mL/min/1.73m², metformin is contraindicated and should be discontinued immediately, as recommended by the Kidney International guidelines 1, 2, 3

Critical Monitoring Requirements

  • Monitor vitamin B12 levels in patients on metformin for more than 4 years due to risk of deficiency, as recommended by the Kidney International guidelines 1, 2, 3
  • When eGFR <60 mL/min/1.73m², increase monitoring frequency to every 3-6 months, as recommended by the Kidney International guidelines 1, 3

Important Clinical Considerations

  • If glycemic targets are not met with reduced metformin doses in CKD, consider adding a GLP-1 receptor agonist as the preferred alternative agent, as recommended by the Kidney International guidelines 1, 3
  • Implement "sick day rules": temporarily discontinue metformin during serious intercurrent illness that increases acute kidney injury risk, as recommended by the Praxis Medical Insights guidelines 3
