Indications and Management of Trelegy Ellipta Triple Therapy for COPD

Patient Selection and Indication

  • Adults with severe chronic obstructive pulmonary disease who continue to experience repeated exacerbations despite optimal dual bronchodilator therapy should be considered for triple therapy with fluticasone furoate/umeclidinium/vilanterol. 1

Administration and Technique

  • Trelegy Ellipta is delivered via the ELLIPTA dry‑powder inhaler; proper inhaler technique must be verified before initiating or escalating therapy. 2
  • Approximately three‑quarters of COPD patients make critical errors with inhaler use, underscoring the need for technique assessment prior to therapy escalation. 2
  • A confirmed diagnosis of COPD and exclusion of alternative respiratory conditions are required before starting triple therapy. 2
  • Current inhaler technique should be reassessed before attributing treatment failure to medication inefficacy. 2

Strategies When Standard Triple Therapy Is Insufficient

  • If symptoms persist on the standard dose, referral for home nebulizer therapy with high‑dose bronchodilators should be considered after formal respiratory specialist evaluation. 2
  • Addition of theophylline may be contemplated in severe disease, with careful monitoring for adverse effects. 2
  • Exacerbation management should be optimized using increased bronchodilator dosing, short‑course oral corticosteroids (e.g., 30 mg daily for one week), and antibiotics when indicated. 2
  • Non‑pharmacologic interventions—including pulmonary rehabilitation, nutritional support for underweight patients, and vaccination—should be incorporated into the comprehensive care plan. 2
