Management of Dry Persistent Cough

First-Line Approach

  • The American College of Physicians recommends starting with simple home remedies like honey and lemon mixtures before pharmacological treatment, as they are often as effective as medications for dry cough 1, 2
  • Honey and lemon work through central modulation of the cough reflex and provide a demulcent coating effect, making them a suitable first-line alternative to medications 1, 2
  • The optimal dose of dextromethorphan for maximum cough suppression is 30-60 mg, which is higher than standard OTC dosing, according to the American Academy of Family Physicians 1, 2
  • Standard OTC doses of dextromethorphan (10-15 mg) are often subtherapeutic, and the maximum daily dose should not exceed 120 mg, as recommended by the American College of Chest Physicians 1, 2

Alternative OTC Options

  • First-generation antihistamines with sedative properties, like diphenhydramine or chlorpheniramine, can suppress cough and are particularly useful for nighttime symptoms, as suggested by the National Institute for Health and Care Excellence 1, 2
  • Menthol inhalation suppresses the cough reflex acutely but provides only short-lived relief, according to the European Respiratory Society 1, 2

What NOT to Use

  • The British Thoracic Society recommends against codeine due to its poor benefit-to-risk ratio, as it has no greater efficacy than dextromethorphan but has significantly more adverse effects 1
  • Over-the-counter combination cold medications are not recommended until proven effective, as stated by the American College of Chest Physicians 3

When to Seek Further Evaluation

  • The American Academy of Family Physicians recommends stopping self-treatment and seeking medical attention if cough persists beyond 3 weeks, as it may require a full diagnostic workup rather than continued antitussive therapy 1, 2
  • Patients should seek medical evaluation if they experience increasing breathlessness, fever, malaise, or purulent sputum, as these may indicate serious lung infection, according to the National Institute for Health and Care Excellence 1
