Steroid Mouthwash for Oral Lichen Planus

Critical First-Line Treatment Protocol

  • The American College of Dermatology recommends applying high-potency topical corticosteroid gels, such as clobetasol 0.05% or fluocinonide 0.05%, directly to dried oral mucosa twice daily for 2-3 months, as first-line therapy for oral lichen planus 1, 2
  • Gel formulations are mandatory for oral mucosal disease, as they provide superior drug delivery and adherence compared to creams, ointments, or mouthwash formulations 1, 2

Treatment Algorithm Based on Disease Severity

  • For mild to moderate disease, the National Comprehensive Cancer Network recommends starting high-potency topical corticosteroid gel for 2-3 months 1, 2
  • For moderate to severe disease, the treatment protocol includes continuing topical corticosteroids and adding oral antihistamines for symptom control, considering a short course of oral prednisone, and adding narrow-band UVB phototherapy for widespread involvement 1, 2

Alternative First-Line Option

  • The American Academy of Dermatology suggests that tacrolimus 0.1% ointment is an effective alternative when corticosteroids are contraindicated or ineffective 1, 2

Adjunctive Measures

  • The American Dental Association recommends applying compound benzocaine gel topically for severe pain 1
  • The American Academy of Dermatology advises patients to avoid all irritants and fragranced products 1
