Management of Refractory Ascites with Large Volume Paracentesis

Introduction to Refractory Ascites Management

  • The American Association for the Study of Liver Diseases recommends that patients with refractory ascites should undergo large volume paracentesis (LVP) with albumin replacement, as it offers significant advantages over continued diuretic escalation, including faster symptom relief and shorter hospitalization, with a strength of evidence level of high 1, 2, 3

Diuretic Dosing and Refractory Ascites Criteria

  • The maximum doses for spironolactone and torsemide/furosemide are 400 mg/day and 160 mg/day, respectively, and should be trialed for at least 1 week on salt restriction <5 g/day before declaring treatment failure, with a strength of evidence level of moderate 1, 2, 3
  • Diuretic-resistant ascites is defined as a mean weight loss <800 g over 4 days despite maximum doses, or urinary sodium output less than sodium intake, with a strength of evidence level of high 1, 2

Clinical Benefits of LVP

  • LVP with albumin replacement offers faster symptom relief, occurring within minutes to hours, compared to days to weeks with diuretics, with a strength of evidence level of high 3
  • LVP significantly reduces the length of hospital stay compared to diuretic therapy alone, with a strength of evidence level of moderate 1, 2

Albumin Replacement Protocol

  • For volumes >5 liters, mandatory albumin replacement at 6-8 g per liter of ascites removed is recommended to prevent post-paracentesis circulatory dysfunction (PICD), with a strength of evidence level of high 1, 2, 5

Post-LVP Management

  • Diuretics must be reintroduced within 1-2 days after LVP to prevent rapid reaccumulation of ascites, which occurs in 93% of patients without diuretics, with a strength of evidence level of high 4, 6
  • The recurrence of ascites drops to 18% with the use of spironolactone after LVP, with a strength of evidence level of moderate 4, 6
