Oral Hydralazine Dosing Guidelines

Introduction to Oral Hydralazine

  • The American College of Cardiology recommends an oral hydralazine dose of 25-50 mg as an appropriate substitute when IV administration is not feasible 1, 2

Dosing Recommendations

  • For acute management when IV administration is not possible, the initial oral dosing range is 25-50 mg, corresponding to the initial oral dosing range recommended in heart failure guidelines 1, 2, 3
  • Typical oral dosing for ongoing management is 25-50 mg three to four times daily, with a target total daily dose ranging from 100-300 mg divided into multiple doses 1, 2, 3
  • The mean total daily dose achieved in clinical trials for heart failure was 175 mg 1

Clinical Considerations

  • Blood pressure begins to decrease within 10-30 minutes after IV administration, with effects lasting 2-4 hours, and oral administration will have a somewhat delayed onset compared to IV administration 4, 5
  • The unpredictability of response and prolonged duration of action make hydralazine less desirable as a first-line agent for acute hypertension management 4, 5

Practical Administration Guidelines

  • For acute management, 25-50 mg oral hydralazine may be repeated every 4-6 hours as needed, not exceeding 300 mg total daily dose 1, 3, 4, 6
  • For transition to maintenance therapy, convert to a regular dosing schedule of 25-50 mg three to four times daily, adjusting based on blood pressure response and tolerability 1, 2, 3
