Operative Time for Partial Mastectomy with Mastopexy

Procedural Context and Timing

  • The National Comprehensive Cancer Network recommends that volume displacement techniques using mastopexy be performed during the same operative setting as the breast-conserving lumpectomy by the same surgeon who is performing the cancer resection, combining the removal of generous regions of breast tissue with mastopexy techniques to fill the surgical defect 1, 2, 3
  • This approach allows for the removal of breast tissue and the shifting of remaining breast tissues to fill the surgical defect, and is typically performed by the same surgeon who is performing the cancer resection 1, 2, 3

Technical Factors Affecting Operative Time

  • Meticulous hemostasis is of critical importance in partial mastectomy with mastopexy, as hematoma formation can produce changes difficult to interpret by physical examination and makes mammography interpretation difficult, and this attention to hemostasis necessarily extends operative time but is essential for optimal outcomes, as recommended by the American Cancer Society 4
  • The primary focus should be on treatment of the tumor, and this treatment should not be compromised when decisions regarding breast reconstruction are made, and attempting to minimize operative time at the expense of adequate tumor margins or proper hemostasis will lead to worse outcomes, as recommended by the National Comprehensive Cancer Network 1, 4
