Management of Mixed Dementia with Vascular Cognitive Impairment and Possible Lewy Body Dementia

Diagnostic Formulation

  • The American Academy of Neurology recommends that a patient meets criteria for dementia based on a MoCA score significantly below normal cutoff, requiring assistance with all IADLs and moderate ADLs, and having behavioral changes with apathy and depression, as seen in this patient with a MoCA score of 17/22 4, 3
  • The diagnosis of mixed etiology dementia with vascular cognitive impairment and possible Lewy body dementia is supported by moderate to advanced chronic microvascular ischemic disease on CT and prominent visual hallucinations for 5 years with increasing frequency 1, 2

Immediate Management Priorities

  • The American Academy of Sleep Medicine recommends initiating CPAP therapy immediately for documented sleep apnea to improve hallucinations, depression, cognitive function, and ADL ability 1, 2, 3
  • The American Geriatrics Society recommends implementing a comprehensive fall prevention program immediately, including physical therapy evaluation, occupational therapy, and removal of fall hazards, given multiple daily close-call falls and recurrent closed head injuries 4, 5

Pharmacologic Treatment for Dementia

  • The American Academy of Neurology recommends starting memantine for moderate to severe dementia, as it improves daily functioning and cognitive performance 6
  • The American Psychiatric Association recommends considering a cholinesterase inhibitor, such as donepezil, if hallucinations worsen or cognitive decline accelerates, particularly if Lewy body dementia is confirmed 6, 2

Neuropsychiatric Symptom Management

  • The American Psychiatric Association recommends initiating an SSRI for depression and apathy, as depression is common in dementia and may have preceded cognitive impairment 6, 5
  • The American Academy of Neurology recommends not using typical antipsychotics for hallucinations without specialist consultation, given the high likelihood of Lewy body dementia, and considering low-dose quetiapine only if hallucinations are distressing and non-pharmacologic measures fail 2

Specialist Referrals

  • The American Academy of Neurology recommends referring to a dementia specialist for atypical presentation with prominent hallucinations, as hallucinations as a prominent early feature suggest Lewy body dementia or other atypical dementia syndromes 1, 2, 5
  • The American Academy of Sleep Medicine recommends referring to sleep medicine for CPAP initiation and management 3

Monitoring and Follow-Up

  • The American Academy of Neurology recommends scheduling follow-up every 3-4 months initially, given behavioral symptoms and rapid decline, and reassessing cognition, functional status, and neuropsychiatric symptoms regularly 5, 7
  • The American Geriatrics Society recommends assessing fall frequency and circumstances at each visit and evaluating caregiver burden and need for additional support services 5, 7
