Insulin Conversion and Pharmacokinetics

Understanding Insulin Formulations

  • U-500 regular insulin is five times more concentrated than standard U-100 insulin, with distinct pharmacokinetics, similar onset, but delayed, blunted peak effect, and longer duration of action, functioning more like an intermediate-acting insulin 1, 2, 3
  • Toujeo (Insulin Glargine U-300) is three times more concentrated than standard U-100 insulin glargine, with a more stable and prolonged pharmacokinetic profile 2, 3

Dose Comparison and Clinical Application

  • U-500 at 30 units TID equals 450 units of U-100 equivalent, while Toujeo 85 units of U-300 equals 255 units of U-100 equivalent, delivering significantly different amounts of insulin 1, 3
  • U-500 provides both basal and prandial coverage due to its intermediate-acting characteristics, whereas Toujeo provides primarily basal coverage with minimal prandial effect 4

Safety Considerations

  • Concentrated insulins, such as U-500, carry an increased risk of dosing errors if not properly prescribed and administered, and should be prescribed with dedicated U-500 syringes to minimize errors 5
  • Toujeo, available only in prefilled pens, minimizes the risk of dosing errors 3
